- Full Name
- Subject heading with the game in question
- Detail your issue/problem
- Platform the game was purchased on
- Country the product was purchased in
- DLC/Code (if applicable) that is not working
- Code (if applicable) given by retailer
- A clear proof of purchase - photo/scanned copy
